| Payment Policy Fees for session classes (one hour once a week) are $55/month for the first child, $45/month for the second child and $35/month for the third child plus membership. Membership dues are $25/annually for one child and $55 for a family membership. If you pay by the first day of the month, you will receive a $5 discount per child. Withdrawal Policy Thoroughbred requires their customers to give a two-week written notice to withdraw from classes should they choose to withdraw before the end of the session. Please understand Thoroughbred strives to provide a top quality program with low student teacher ratios. To be able to afford this quality program, you agree when signing this waiver to honor our withdrawal policy. Make-up Policy Thoroughbred provides a place for your child; therefore, missed classes cannot be deducted from tuition costs. If your child misses class due to an excused absence such as illness, school function etc., Thoroughbred will make every effort to schedule a make-up class. There is a $1 charge for each make-up class scheduled. The $1 fee also applies to those students that choose Friday open gym as an alternative make-up. I have read and understand the above payment, withdrawal, and make-up policies for Thoroughbred Gymnastics. |

| WARNING OF RISK Gymnastics, cheerleading, tumbling and trampoline are inherent risky sports. No matter how careful the students and instructors are, no matter the number and experience of the spotters, no matter what height is used or the landing mat, the risk cannot be eliminated. Reduced, yes, but never eliminated. The risk of injury includes bumps and bruises and more serious injury such as broken bones, dislocations and muscle or ligament tears. The risk also includes catastrophic injuries such as permanent paralysis or even death from landings or falls on the back, stomach, and head. Thoroughbred does not carry medical insurance on their students. By participating in our program Thoroughbred assumes that the customer is covered by their own medical insurance or is financially responsible for any accident incurred by the student or customer on the premises of Thoroughbred Gymnastics Inc. or under the care of a Thoroughbred employee. I have read the above information and understand the inherent risk of injury when participating in an activity at Thoroughbred Gymnastics Inc. I will discuss the importance of listening and following the direction of the staff with my child. |
